If someone marries a USC and is already here on another visa, do they have to return to the UK to wait for the process to complete? and how long would this take?

If they entered on a Visa allowing Dual intent, H or L, no issue.
If they entered on any other (Crewmans excepted) they can adjust if they did not have immigrant intent on last entry.

With very few exceptions-such as crewman's visa or if you dumped your original K-1 or K-3 sponsor (or vice versa) you can adjust status in the US.
If you a re illegally present you should definitlely do that and avoid leaving the country
